If you are on the Dental HMO/Managed Care plan, under “Which is Your Plan” select Met355 in the drop down and locate the facility number. You must enter the facility number of a general dentist at the time of enrollment or you may select a provider through your MyBenefits account. If you need to see a specialist, your general dentist will provide you with a referral. You must select a provider by the 20th of the month to be on the next months roster.
Pro Tip: The PPD network allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PDP network.
